Request Investigations: Hospitals, Doctors, Nurses, etc.

As many of you know our healthcare system is in shambles.  If you have an issue or concern regarding hospitals, doctors, nurses, ambulatory care, etc. in the USA you may request investigations or assistance by contacting the following agencies:

The Joint Commission: an independent not-for-profit organization accredits and certifies more than 18,000 health care organizations and programs in the United States.  http://www.jointcommission.org/

In addition, you may contact The Centers for Medicare & Medicaid Services for assistance with Medicare, Medicaid, EMTALA (Emergency Medical Treatment and Labor Act), etc. http://www.cms.gov/

For those of you in alternate states, you may want to use this information as a guide, contact your local elected officials, and/or local health dept.

The following is an update regarding how to request investigations involving hospitals, doctors, nurses, state agencies, etc. in the New York State Area.

If you need to investigate a NYS HOSPITAL regarding patient care, grievance or concern:

NYS Department of Health:
NYS DOH
Hospital Program, 15th Floor
90 Church Ave.
New York, NY 10007
http://www.health.state.ny.us/nysdoh/hospital/

Once an investigation has been processed you can request a “Statement of Deficiencies” and after a few weeks you should then request a “Plan of Corrections” through the Freedom of Information Legislation or FOIL.  Please email FOIL@Health.state.ny.us with your request.

If you need to investigate a NYS DOCTOR regarding patient care, grievance or concern:
Office of Professional Medical Conduct
212-417-4900
800-442-8106
800-663-6114

If you need to investigate a NYS NURSE regarding patient care, grievance or concern:
NYS Education Dept.
212-951-6400

Finally, if you have an issue with a NYS agency (such as the NYS DOH) please contact the Inspector General’s office at: 1-800-367-4448 or www.IG.state.ny.us

I hope this information is helpful in your quest for help.  Please feel free to contact me if you have any questions, or if information has changed over time so I may update it for others who are in need of assistance.

The preservation of health is a duty. Few seem conscious that there is such a thing as physical morality.
